14 Ventilator and blood oxygen?

English Transcript (中文全文在下方阅读)

AMANDA: Alright Dr. Lio, we just have a few questions left. One of our community members heard that some patients do not do well even on respirators. They are wondering: why do some patients continue to have low blood oxygen saturation despite being on ventilators? Did those patients pass away due to an intrinsic problem with their blood in addition to being infected with COVID?

DR. LIO: So, oxygen has to be able to go from your mouth into your lungs, and from your lungs into the blood itself. There are different barriers that can get in the way of that. If you have fluid in those air sacs within the lungs, the oxygen can get up to that point, but it is not going to easily pass that fluid into the bloodstream. So you can imagine, even though you are on a ventilator, that ventilator is pushing air, pushing extra oxygen into the lungs. If there is a lot of fluid in the lungs in the first place, it doesn’t matter how much oxygen you push in, it’s not going into the blood. Something else that might happen is that even if you have that air sac and it’s open, oxygen is getting there, there might not be enough blood flow going to that part of the lung. So, if the blood flow is going to the part of the lung that is not getting much oxygen, then you’ll still have problems with having oxygen in the blood. When you don’t have enough oxygen in the blood, and all of your tissues require that in order to survive, that’s when things will start getting worse for you.
